Heaved sidewalk on South Park Ave blocks wheelchairs

A long stretch of sidewalk on South Park Avenue has heaved and cracked badly, with slabs lifted several inches by tree roots. A neighbor who uses a wheelchair has to go into the street to get around it, which is dangerous on a busy road. The corner curb cut is also broken. Requesting repair and a proper ADA-compliant ramp at the corner. Posting so others in the neighborhood can confirm the spots that need fixing.
