Heaved sidewalk on a busy street blocks wheelchairs

A long stretch of sidewalk has heaved and cracked badly, with slabs lifted several inches by tree roots. A neighbor who uses a wheelchair has to go into the street to get around it, dangerous on a busy road. The corner curb cut is broken too. Requesting repair and a proper ADA ramp. Posting so others can confirm the spots that need fixing.
