Heaved sidewalk on a busy road blocks strollers and wheelchairs

A long stretch of sidewalk has heaved and cracked badly, with slabs lifted several inches by tree roots. A neighbor who uses a wheelchair has to go into the road to get around it, which is dangerous. The corner curb cut is broken too. Requesting repair and a proper ADA ramp. Posting so others can confirm the spots that need fixing.
